Alhamdulillah pukul 9.30am tadi berakhir ler exam long case aku…hmmm aku rase aku dah buat yang terbaik tadi, hopefully bukan aku sorang jek lah yg rase mcm tu ^^
aku dapat case chronic rheumatic heart disease (^.^)..syukur sgt dpt kes tu coz aku ade cover jugalah time study..sblom tido smlm pun tergerak nak study slide dr KWT yg rheumatic heart tu..hehe tak sangka dpt kes tu plak :)
examiner alhamdulilah dpt dr KWT & sorang dr unimas…2-2 pun ok ler takde ler interuppt or tembak berabis…alhamdulilah mmg bersyukur ler sgt..
juz curious je whether girl tu ade murmur lain selain pan systolic murmur, coz aku confirm mmg aku dgr PSM at mitral area then radiate to axilla & back..so confirm2 tu PSM & the cause shud be mitral regurgitation kan..
tapi td after present finding, dr unimas tu pegi examine patient tu & suruh aku auscultate smula ntuk confirmkan finding aku..hoo time tu aku dh a bit panic sbab slalunya klau kene recheck, msti ade yg tak ngam dgn finding aku tu..
aku pun auscultate ler, tp still dgr bnda yg sama..dr plak tak tnya lebih trus proceed pegi diagnosis & discussion..
so aku yakin je lah jawab Chronic rheumatic heart disease with mitral regurgitation and not in failure.. dr mcm accept jek dx yg aku bgi tu..differential pun dr tak tnye ^^
then trus dr tnya psal criteria RHD yg Jone’s criteria tu..hee nasib ler dah study smlm, so masih segar ler dlm ingatan.. bdak tu ade 3 major criteria out of 5 (carditis, migratory polyarthritis & erthema marginatum…yg chorea & subcutaneous nodules tu dia takde)
pastu dr tnya pasal antibiotic prophylaxis, coz bdak tu mmg on Penicillin..dr tnye smpai biler harus bagi antibiotic tu..klau ade carditis shud be given life long & klau xde carditis juz bgi till age 21 0r lepas 5 tahun takde manifestation of the dz..
so, tu jek lah yg aku kene tnya td…takde plak dr tnya psal ix & treatment..padahal dah berabis aku bersedia nak jwb hihi..habis je trus dr blah, aku plak yg blur coz xsure dah btol2 habis or still ade discussion lgi, yelah rase macam sekejap je kan..skalinye mmg betul2 dah habis derrrr~
yeaahhh aku rase relief sgt!! aku rase aku dah buat yang terbaik tadi..harap2 semuanya ok jek lah…
ape2pun aku serahkan segalanya pada Allah swt..
semoga untuk short cases esk aku dapat kes yg ok, examiner yg ok & hopefully aku boleh buat yang terbaik & tak panic attack (-.-)
