wohooo akhirnya pada 1 sept 2012 aku merdeka secara rasminya ntuk 2 bulan sblom perlu menghadap another exam ~.~

so hows my last exam short cases??

hmm kalau long case aku dpt 1st rotation, ntuk short case plak aku dpt rot 4..phew so aku ade masa la lagi ntuk study ape yg patut kan..

ekceli malam sblom short case tu aku juz spent time praktis PE dgn dorang ra..mule aku bajet praktis kejap je kan..skali ambik kau 3 jam siot..haha but berbaloi la coz byk jga yang aku blajar kan esp tang neuro examination..hoho sebab spjg aku bergelar med student, aku xpenah dpt kes neuro dlm exam..

tp ntuk exam kali ni, aku rase mcm aku akan dpt je..tsk~ dan ianya mmg betol2 berlaku..lol ni mesti sebab aku dh ade “tendon hammper” sendiri..that’s why ler..haha coz sblom ni aku juz pnjam dgn org je.. (haiz tringat yg aku patahkan TH milik chalen masa 3rd year hari tu, hee gommene chalen, time nak masuk 5th year baru terganti ^^)

so, balik je dari praktis PE tu, aku ingat nak bace2 skit la kan psal management diseases tu sume, skalinye balik bilik aku bantai mkn megi smbil2 on9~ haizz mmg susah nk mengawal nafsu ni kan =.=

huh mlm tu mmg tido dlm keadaan yg nerves ler coz rase mcm ade je lagi bnda yg aku tak sempat nak cover..aku just harap aku dpt kes yg ok & examiner yg ok je la..

so here i am..on the day of exam..

dup dap dup dap dup dap…arghhhh nervousnye~~~ aku ni dah ler jenis yg mudah panic attack..kene kuarantin plak dlm bilik yg super kecik..boleh dpt brain hypoxia kot..nasib je half of us pegi stay bilik lain..so still boleh gasping for air ler..haha over plak..

erm masa kuarantin tu ade jugalah diskus2 skit kes dgn chalen..pastu dgar dorang keenam sume berdiskus..tapi to tell the truth, aku sbnarnya dh tak boleh nak absorp ape dah..makin bace makin blur, makin dgr makin stress..mane taknye, dorang kbykan diskus bnda yg super high class.. aku plak mcm “eh? sejak biler kite ade blajar pasal tu sume?? apsal aku tak tau?” LOL!!

p/s: tu lah time org busy study ko buat ape nurul?? busy on9, merayau, mengelamun…hahaha

1st case

wohooo syukur sgt2…mcm tak percaya jek aku dpt kes bdak rheumatic heart dz tu..iye, bdak yg aku dpt time long case tu.. aku masuk je dlm bilik examiner tu, 1st smile sebab dpt grandma & dr unimas as examiner (kurang skit nervous) then 2nd smile biler trpandang bdak tu.. (oh adik, kite bertemu kembali..hehe suke aku tgk bdak tu..cantik kot, muke mcm nia ramadhani..sporting plak tu..thanks a lot ye Azlin :)

as expected, aku disuruh ntuk examine cardiovascular system..mcm biase lah aku buat… then masuk part discussion, aku ckp ler dx aku Mitral regurgitation secondary to rheumatic heart dz.. ekceli dr unimas mcm pelik je coz aku sebut tang RHD tu..msti dia suspek aku tau kes ni ^^ so dia tanya lgi another cause..haizz aku xdpt fikir ler plak..then dia tnye psal investigation..aku ckp je lah buat ecg, cxr & echo..

last part dr tnye do u thing this pt got carditis? haiz aku boleh blur ckp xde..tpi kan aku rase ader lah coz dah terang2 ade murmur tu sume..haizz bengong je..dr tnye lgi ape sign carditis..haha aku juz dpt jwb few je..then last dr tnye what do u want to advice this pt since she has a heart problem? adeh aku ekceli xbrape fhm soalan dr..somemore aku confius whether perlu advice base on rheumatic heart dz or wat..haizz mmg tak terjawab pun soalan tu..

2nd case

Nah kan~ what did i told u guys…i really got neuro case!!! OMG!! mmg nervous ler~ tp buat2 cool je la & try ntuk menenangkan diri..

aku mase start examine tu aku confius whether bdak tu ade cp or dmd..obviously bdak tu ade m/s wasting both lower limbs at calf m/s, spastic hypertonia, hyperreflexia, m/s power 4/6, no clonus and no babinski (tak pasti ler klau aku yg xdpt elicit)

ekceli klau skali bace aku pnya description boleh trus terfikirkan CP..tp mase exam tu, aku jadi confius biler bdak tu punye muscle power mcm intact..aku plak asyik terfikir yg klau bdak cp slalunye m/s power mmg kurang kan (kebanyakan yg aku tgk kat ward ler)..

so aku mmg confius nk ckp dx yg mane 1 ni..yg part m/s wasting tu tak pasti mana lgi prominent either calf or quadriceps..

masa aku describe findings tu dr unimas tu tnye lah which part lgi prominent..aku siiiiikiitttt je lgi nak sebut quadriceps, nasib dr dulu sebut calf..so ok~ bdak ni confirm cp la..so finding yg lain aku sebut mcm biase lah..cuma part babinski tu lah dr mcm tak setuju aku ckp negative..(dah mmg mase aku buat tu mmg negative..camne juga~) dr plak ingat aku xpaham konsep babinski tu..huuu, nasib ler dpt jwb biler dia tnya sign ntuk babinski +ve & -ve..

pastu aku terkene part clonus tu..duhhh dr komen yg cara aku buat clonus salah x_x tapi aku rase aku buat mcm yg dr slalu ajar, and time praktis pun dorang ajar mcm tu..aku plak takut nak defend..so aku pun pasrah je lah..huhu hrp2 tak effect sgt ler dgn markah (T^T)

so dx aku ntuk kes ni, spastic diplegic CP with UMNL.. dr seem to be satisfied.. pastu time’s out!! omg dr xsempat nak tnye management..juz smpat tnya common cause of spastic diplegic CP..nasib ler dpt jwb =.=..

pastu grandma yg dari td senyap je trus ckp “ok thank you nuuruu farahana” hoho so my exam officially over for now~~~~

huhu tp risau ler jugak coz tak diskus pun management kan..huhu hopefully ok2 lah

akhirnye tamat ler perjalanan kami slama 7 minggu dlm paeds posting..

for me, aku takde ler brape enjoy sgt posting ni..aku rase masa 4th year dulu lagi happening kot..maybe sebab final year kan..so it getting more & more serious..tp aku rase sebab aku rindu dgn gp 4th year aku :(

hmm after this i shud well prepare mentally & physically coz gonna entering a tough posting…
